Application for Admission (2010 – 2011)

報名須知  Information on Application

1. 遞交申請表地址：香港黃竹坑南風道9號港大同學會書院。
   The completed application form should be submitted to:  HKUGA College, 9 Nam Fung Road, Wong Chuk Hang, Hong Kong.

2. 遞交申請表時，請附下列證明文件之影印副本。
   Please enclose copies of the following documents:   

   1. 學生的香港身份證／護照
      Student’s H.K. ID Card / Passport
    
   2. 學生派位證或小六學生證
      Secondary School Places Allocation Form (SSPA) or Student’s Primary Six Identification Card
    
   3. 學生最近兩年的成績表
      School reports of the last two years
    
   4. 其他附加資料（如證書或獎狀等，尺寸不超出A4） 亦可交予本校。
      Other relevant documents (certificates and awards not bigger than A4 size) could also be attached

3. 遞交申請表請連同HK$100報名費之劃線支票（報名費恕不退回，抬頭請書「港大同學會書院」），支票背面須寫上申請人姓名。
   A non-refundable application fee of HK$100 by crossed cheque (made payable to “HKUGA College”) with the applicant’s name at the back of the cheque should be submitted with this application form.

4. 附上2個分別貼上$1.4郵票及寫上申請人姓名地址之回郵信封。
   Enclose 2 self-addressed and stamped (each HK$1.4) envelopes with the applicant’s name and address.

5. 面試時間及地點將另行通知。
   Time and place for interview will be announced later.

6. 獲得取錄之申請人將由學校書面通知。
   The school will notify successful applicants in writing.

7. 二零一零至二零一一年度中一學費為全年三萬五千元（有待教育局批准）。
   Annual school fee for Secondary 1 (2010-2011) is $35,000 (subject to EDB approval).

8. 根據 «個人資料(私隱)條例 »，你有權查閱或更改所提供的個人資料。
   According to the Personal Data (Privacy) Ordinance, you have the right to access and amend personal data provided in the application form.

9. 如須查閱或更改個人資料，請以書面提出並寄往港大同學會書院。
   Requests for access to and amendment of personal data should be made in writing and sent to HKUGA College.
